2. Where one or more services in addition to those indicated in the lease form for a dwelling are offered, including services of a personal nature to be provided to the lessee, the lessor must complete, in addition to the appropriate schedule of the mandatory form of the Administrative Housing Tribunal, the form appearing in Schedule 6.
In the case of services of a personal nature to be provided to the lessee, the lessor must indicate the cost of each such service and the total rent payable.
2. Where one or more services in addition to those indicated in the lease form for a dwelling are offered, including services of a personal nature to be provided to the lessee, the lessor must complete, in addition to the appropriate schedule of the mandatory form of the Régie du logement, the form appearing in Schedule 6.
In the case of services of a personal nature to be provided to the lessee, the lessor must indicate the cost of each such service and the total rent payable.
2. Where one or more services in addition to those indicated in the lease form for a dwelling referred to in Schedules 4 and 5 are offered to a lessee by reason of his personal condition including his age or a handicap, the lessor shall also use the form of the Régie du logement appearing in Schedule 6.
